In addition to being fully trained in cardiology, a cardiac electrophysiologist does two extra years of training, specializing in heart rhythm problems of the heart. Q: What are the types of heart rhythm problems that you see in practice? Rhythm problems are divided into: 1) slow heart rate called “bradycardia”and 2) fast heart rate called “tachycardia” and 3) Skipped beats that can be earlier beats coming from top chamber (premature atrial contraction) or lower chamber of the heart ( premature ventricular contraction). Q: What are the symptoms that patients can experience? People with bradycardia usually presents with fatigue, dizziness, shortness of breath and passing out. The treatment can be as simple as adjusting some of the medications. In some cases; however, a pacemaker is recommended to help maintain a normal heart rate and avoid passing out. People with tachycardia usually presents with palpitations, shortness of breath, chest pain, dizziness and passing out. Patients with skipped beats often presents with similar symptoms. These symptoms can be very scary to the patient and it can sometimes lead to anxiety. It is also important to mention a common form of tachycardia called atrial fibrillation. It is quite irregular, usually fast and does carry increased risk of stroke. Patients with atrial fibrillation often require blood thinning medications to lower their risk of stroke. Q: How do you treat the heart rhythm problems? There are different ways to treat rhythm disturbances. In some cases, reassurance and close observation will be enough. In other cases, we use medications like a beta blocker or calcium channel blocker that slow down the heart rate AND/OR anti-arrhythmic medications that will change the electrical property of the heart. Frequently, this should help quiet down the rhythm problem. In certain patients, medications are sometimes ineffective or they can produce side effects. In this situation, we might resort to Electrophysiology study and ablation therapy. We also use device implantation as a way to detect and treat rhythm problems. These devices include insertable loop recorders, pacemakers and defibrillators. In patients with heart failure, we implant specific kind of defibrillator, called biv-defibrillators that will help symptoms of heart failure and it can prevent sudden death due to dangerous rapid rhythms.
